The Family Portal is an extension of InteliChart’s market-leading Patient Portal and empowers family members and caregivers to stay informed about the health and well-being of loved ones who are in long-term care, skilled nursing, home health and senior living facilities. Family Portal provides insight that eases the challenges of managing the health of a loved one from a distance.

Health Status Monitoring

Health status, medication adherence, meal consumption, upcoming medical appointments, and participation in activities can all be monitored through the Family Portal. Family members and caregivers have secured access to a resident’s account for staying current with their daily health and can secure message staff members electronically with questions and concerns, eliminating the frustration of phone tag.

Care Calendar Access

Families can view and schedule activities for their loved one - doctor visits, podiatry appointments, haircuts, special activities, and social events - through Family Portal’s care calendar.

Staying in the Know

Family members can stay current with their loved one’s meal consumption with trend graphs that indicate what was consumed and the quantity. They can monitor medications, staying abreast of what loved ones are taking, when they are administered, dosages, refusals, and tolerance. Family Portal also allows family members to keep tabs on outstanding invoices and make payments electronically.
